It is with mixed emotions that I am announcing my resignation from my current medical practice at the High Plains Family Medicine – Borger Medical Clinic, effective July 31, 2016. It has been a great pleasure providing for your health care needs over the past 5 years. I will be starting a new chapter in my career at another clinic site in Amarillo, TX.
As of August 1, 2016, Dr. Dustin Hawley will be taking over my portion of the practice and will be the sole director of High Plains Family Medicine – Borger Medical Clinic.
In addition to Dr. Hawley, you will continue to receive excellent medical care from the other providers including Bake Bannister, PA-C and Wendy Kuehler, APN. I am also proud to announce the addition of another mid-level provider, Kaitlyn Banks, who is joining the practice in the Fall of 2016.
Of course, you may seek medical care from another doctor if you like. If you choose to do so, I recommend looking for a new physician as soon as possible.
Your medical records are confidential, and a copy can be transferred to another doctor or released to you or another person you designate only through your permission. If you plan to continue with this office, you are already considered an established patient and you will not be required to re-establish as a “new” patient. If you choose to see a different physician, please contact our office as soon as possible to complete the necessary release forms to have your records copied or transferred. Until then, your records will remain on file at the High Plains Family Medicine – Borger Medical Clinic: 104 N Bryan, Borger, TX 79007.
